The forms necessary for your child’s enrollment can be found below. Please download and print the required paperwork and return the completed information to the school on or before your child’s first day in attendance. All Required Forms, including the application and tuition contract previously submitted to the school, must be in your child’s file for it to be complete. If your child is returning to Fairview, you must update the forms marked with an *asterisk. Note: Emergency Cards will be sent to new students before school starts. At any time, if there is a change in information, notify us immediately and request to update your emergency card.
